If you would like to coach within the CMYGSA, you must have a current background check that is not dated older than 3 YEARS with the CMYGSA prior to taking part in any team function on the field as per the Child Protective Services Law.  Please refer to the information above on the new laws effective 12/31/2014.

 

This background check includes the following:

  • Child Abuse History Clearance (cost of $10)
  • PA State Criminal Background Check (cost of $10)

If you have NOT lived within the state of PA for the past 10 years:

  • Federal Criminal Background Check (cost of $25.75 and includes getting fingerprints)

 

As per the law above, your clearances must be at least more current than the following dates: 

 

SPRING 2016

12/31/2014

 

FALL 2015

12/31/2014

 

 

 

INSTRUCTIONS for obtaining your clearances

 

STEP 1 - Child Abuse History Clearance

  1. The fee will be $10 payable by credit card.
  2. Go to the Child Welfare Portal.
  3. Click on Create a New Account (or Login if you have already created an account).
  4. Login to your Account.
  5. Next to My PA Child Abuse History Clearances, click on Create Clearance Application.
    • Here are some answers:
      • Application Purpose = Volunteer
      • Volunteer Category = Other
      • Agency Name = Canon-Mac Youth Girls Softball Association
  6. Complete the application as necessary.
  7. There is no organization payment code, so you will have to pay for the background check.
  8. It should be completed within 14 days.  You will get an email once it's ready.  (If you have NOT lived in the state of PA for the last 10 years, skip STEP 2 for now until you get your Child Abuse History Clearance and continue with STEP 3 below.)
  9. Come back to this site once you get the email with your Child Abuse History Clearance Results and continue following these instructions.
  10. Log back into the site, pull up your results, and save it to your computer.  Once you have this, continue to STEP 2 BELOW.

 

STEP 2 - PA State Criminal Background Check:

  1. The fee will be $10 payable by credit card.
  2. Go to the PATCH website.
  3. Select "Submit a new record check" (you are considered a non-registered user).
  4. Following the steps.
  5. When it asks for Reason For Request - SELECT "Volunteer".
  6. IMPORTANT - at the screen for "Record Check Request Results", be sure to follow these steps:
    • Click on the CONTROL NUMBER next to your name.
    • Click on the link that says "Certification Form".
    • Print this out and save it to your computer to upload to our site - THIS IS THE DOCUMENT WE NEED!!!
  7. Once saved to your computer, it needs to be uploaded to our site along with the results from STEP 1 and STEP 3 (if applicable) via the link below.

 

STEP 3 - Federal Criminal Background Check

  1. ONLY DO THIS STEP IF YOU HAVE NOT LIVED IN THE STATE OF PA FOR AT LEAST 10 YEARS!!!!
  2. The fee will be $25.75.
  3. Go to the Applicant Fingerprinting Online Services website.
  4. Select REGISTER ONLINE and complete the application.
    • For Reason Fingerprinted - Employment with a significant likelihood of regular contact with children
  5. This will take you through the process to register, pay and get a RECEIPT to take to the Fingerprint site.
  6. Take to the fingerprint site: License and proof of transaction (receipt)
  7. The closest fingerprint location is:
    • Mail, Boxes & More
    • 123 Washington Street
    • Washington, PA 15301
    • 724-222-7660
    • Email: mboxmore201@hotmail.com
    • Hours for fingerprinting:
      • Monday – Friday – 9 am to 5 pm
      • Saturday – 10 am to 12:30 pm
  8. You should get the results within 14 days.
  9. Once you get your results, it needs to be uploaded to our site.
  10. After your fingerprints have been submitted, do not contact the fingerprint site. You can check your “proof of print” on the Cogent website. Click on Proof of Transaction (Receipts).
  11. Once you have these results, your PA Criminal Record, and your Child Abuse History Clearance, then submit these clearances via the link below.

GUIDELINES:

We will follow Title 23, CHAPTER 63 on CHILD PROTECTIVE SERVICES set by the State of Pennsylvania in terms of what is acceptable on your background check and whether you will be cleared to coach within the CMYGSA.

NOTE:  Please refer to section 6344 (c) titled "Grounds for denying employment or participation in program, activity or service."
